The above form will be discontinued in the future. Please register for our new system, Patient Services, at https://www.patient-services.co.uk/login
Orders can be made by using the prescription re-order form (right hand side of your repeat prescription) and leaving it in the repeat prescription box as you enter the surgery.
Telephone orders can be made between 09:00 - 12:00 daily. Press no.1 on your keypad and your call will answered by automated voicemail. When prompted leave your name, address, telephone number and details of your prescription remembering to spell any difficult words.
Alternatively you can now order online by following the link at the top of this page and completing a simple registration. We are encouraged by the number of patients who have registered for vision on-line services for ordering repeat medication & booking routine appointments.
Please allow 24 hours for prescriptions to be processed and ready for collection and remember to take weekends and bank holidays into account.
Emergency / Acute Prescriptions
All emergency / acute prescriptions ordered before 11.00 may be collected from the surgery after 14:00 and those ordered after 11.00 may be collected after 16:00. Any prescriptions not collected within 7 days of being ordered are destroyed and will have to be re-ordered if necessary. No prescriptions are issued after 17:00 unless for an emergency.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse every six months to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
